We reported a case of 23-year-old premegravida in a spontaneous pregnancy who was seenwith markedly enlarged, multicystic ovaries, abdominal pain, hypervolemia and ascites at 15thweek of gestation. Due to non-iatrogenic background of symptoms, a diagnosis of spontane-ous ovarian hyperstimulation syndrome with pregnancy was established and successfully tre-ated with a conservative approach
